Costs of Service
The costs for personal case management services are privately paid by contracting company/individual. In time, this may/will actually save money because through coordination of utilization management, the necessity of your needs are constantly evaluated making sure that each service provided is needed and well-afforded. Those that are not necessary will be made known and the client makes the choice on having it or not. Either way, the client is well aware of options with choice ultimately in their hands.

Initial Assessment/Care Plan Development.....................................................………….............$350.00 (initial fee)
Hourly Rate for Established Services ………………………………..............................................$ 55.00
Contractual Monthly Service........................................................................................................$220.00/month
(Includes weekly 1 hour service, follow-up calls, monthly status updates to family-unused hours will be banked and used as needed)

Contacting health care providers (insurances, doctors, home services)...................................$ 55.00
Accompaniment to appointments/procedures...........................................................................$ 65.00
Medical Record Review (legal/personal purposes)...................................................................$ 55.00
Research of facilities and coordination of transition..................................................................$200.00
(examples: assisted living, nursing homes, rehab, LTACH)
Utilization Review/Research.......................................................................................................$ 45.00
Coordination of Long Distance Services....................................................................................$200.00
If you decide that you will be needing this service quite regularly, it may be to your advantage to get on monthly payment plans. Prices for this type of set-up are based on weekly interventions and frequent changes made to plan of care; anything excessive of that can be negotiated. Plans aren’t concrete until agreed upon by client with full understanding and a contractual agreement is signed.
